Enable Me Physiotherapy is committed to ensuring that your privacy and
personal information is protected. Information you share with us will be kept
confidential unless you indicate otherwise, or in the event of us having concerns about an
individual’s safety and welfare. We ask you to consent to us processing
information about you, by which you can be identified, to provide our
services to you. In line with GDPR regulation (2018) we are requested to
comply by informing you of:
The type of personal information we collect
We currently collect and process the following information:
• Name, date of birth, GP, address, contact details and details of parents and/or carers
• Past medical history
• Reports or clinic letters from other professionals (EHCP documents, letters, emails)
• Details of assessment, intervention, meetings and communications are recorded in
‘patient notes’ as required by the HCPC.
• Photos and videos of assessment and treatment and for use in personalised
programs

How we get the personal information and why we have it
Most of the personal information we process is provided to us directly by
you in the following ways
• Initial personal details form
• Website contact form on www.enablemephysiotherapy.com
• Emails, texts, phone calls
• Facebook (posts, messages, comments)
• Forms and assessments completed by us (eg postural management
assessment form or aquatic therapy risk assessment form)

We also receive personal information indirectly, from the following
sources in the following scenarios:
• Letters, reports and emails from other professionals or educational
settings

We use the information that you have given us in order to complete
accurate and up to date patient records in line with record keeping
guidance from HCPC and CSP

Personal information is kept confidential. We will never sell or rent personal information.
For us to work safely and effectively, we ask for consent to speak to relevant agencies
involved with a child or adult. These agencies can include, but are not limited to, schools,
NHS teams, education teams, social care teams, professional (employed) carers,
Paediatricians and other professionals involved with the child. Parents and carers of those
with children under 16 years old are asked to provide written consent on the initial personal
details form prior to us speaking to external agencies. Consent can be withdrawn at any
time in writing.

We have a duty of care to report any safeguarding concerns to the relevant professionals
under the Children’s Act 1989 and 2004. We follow the guidance and procedures outlined by
the Dorset Safeguarding Children Board and BCP Safeguarding Children Board to safeguard
children in our care. Whenever possible we will notify you before reporting a safeguarding
concern unless we believe that this would increase the risk towards a child.

We will not distribute or lease personal information to third parties unless we
have your explicit permission or are required by law to do so. We will never
pass on any information we hold about any of our clients to any third party.
We will not use any information we hold on you for any commercial gain. We
will not use any information we hold to use for any unsolicited marketing to
you unless we have your express agreement in advance.

How we store your personal information
Your information is securely stored in line with record keeping guidance
from regulatory bodies. Digital information is password protected on cloud
based software. We also receive information by other means such as
email or facebook, whilst these are not completely secure, when using
these methods to communicate with us, your consent is implied for us to
use these methods of communication with you.

We keep patient records (notes, emails, assessments, photographs,
exercise programs) until a child’s 25th birthday or for 8 years after
discharge from our service if the patient is an adult. We will then dispose
your information securely

Use of cookies
This website uses cookies to better the users experience while visiting the
website. Cookies are small files saved to the user’s computer hard drive that
track, save and store information about the user’s interactions and usage of
the website. This allows the website, through its server to provide the users
with a tailored experience within this website.

this website and its external serving vendors. This website uses tracking
software to monitor its visitors to better understand how they use it. This
